Create a login to our client portal by clicking this link. You may want to bookmark this portal for future access!​

Fill out your demographic information in the portal.  Don't forget to save!

Step 3: request referral from your prescriber
 (for OT, PT, and SLP only if you plan to use insurance)

You can print our form below and bring to your prescriber for them to sign OR you can request they fax us a prescription to (844) 440-2397.
